How to find an NHS dentist
Go to NHS Choices online. Patients are not bound by catchment. Be aware that not all practices will have the capacity to take on NHS patients and may have a waiting list. If after contacting several dental practices you still cant find a dentist accepting NHS patients, you should call NHS England’s Customer Contact Centre on 0300 311 2233 .
Dental Emergency and out of hours care
If you think you need urgent care, contact your usual dentist as some practices offer emergency dental slots and will provide care if necessary.
You can also call NHS 111, who can put you in touch with an urgent dental service.F

Complete the self referral form fully (Found in Reception) and return to the Physiotherapy booking centre at—Warwick Hospital via post, fax email or hand delivery.

Non– Emergency Patient Transport Service
The booking office is open each day between 8am – 8pm Patients are required to have the following information available when booking to ensure an effective process: Name, DOB, NHS number, Pick up and destination address including postcode, GP Name & practice address, Date and time of appointment and type of transport required
